Helpful Information

Facilities Use Request Form (FURF)

This form is necessary for all KML access and activities
Please call first to determine availability and request a link to the online FURF form
FURF Group List (PDF) – please use to list names of all members in your group

Required Forms to Submit for KML Access

Release & Waiver of Liability form (PDF) – please submit annually for all KML access and activities
Health Status Survey – online form to be completed by all guests within 24 hours prior to arrival at KML
Illness Contingency Plan – online form to be completed by PI if staying in KML dorms. Must be approved prior to arrival and all group members should have reviewed the Plan

Boat Trips & Snorkeling at KML

These forms are required prior to any boating activities. Please return with your completed FURF
KML Boat Roster (PDF) – includes Swimming & Snorkeling Skills & Experience Survey
KML Boating & Snorkeling (free diving) Understanding to be reviewed with members of your group and signed by the Principal Investigators or Group Leaders

Seawater Use Request Form (SURF)

Seawater Use Request Form (SURF) – This fillable PDF must be submitted to request space in any KML Seawater System
Call to discuss availability of SW tables with KML staff

AAUS Scientific Dive Reciprocity at KML

Please submit all forms to KML DSO for review and approval at least 14 business days prior to dive mission.

Required Forms:

Letter of Reciprocity form (LOR) – to be submitted for each new dive mission
Release & Waiver of Liability form
Dive Plan Approval form – details of anticipated diving activities
Proof of Insurance for each diver (workers comp, DAN, etc.)
Daily Diver Roster & Air Management Agreement

Sampling and Specimen Collections at KML

Please remember that KML is within the Florida Keys National Marine Sanctuary and adjacent to the Everglades National Park.
You are responsible for providing all necessary permits, special activities licenses, and/or IACUC approvals for all education, research, and specimen collecting.

Self-Captained Vessels for Research Activities

Self-Captained vessel rentals may require an assessment boat trip with a KML captain. Please call to discuss necessary qualifications, planned activities, and availability of vessels.
